Scope of work
Under general direction, integrates cost, quality and utilization to facilitate the admission, continued stay and discharge of the patient. Reviews and evaluates appropriateness of admission or continued stay based on medical necessity. The overall goal of the position is to enhance the quality of patient care and engagement, to promote continuity of care and cost effectiveness through the integration and functions of utilization management, and/or care coordination, discharge planning, and appropriate care transitions. Has accountability for the care coordination and discharge planning of all hospitalized patients.
In collaboration with patient, family, physician, and other health care team members formulates and facilitates the discharge planning activities of assigned patients.
Coordinates care and monitors the utilization of resources for patients, physicians, and third-party payers.
Utilizing the nursing process and critical thinking skills, coordinates the care for assigned patients.
Utilizes established criteria to identify and facilitate the appropriated level of care for assigned patients. Educate patient, family, and health care team members regarding appropriate level of care.
Collects and aggregates data via Allscripts automated case management system and electronic medical record documentation.
Serves as an educator on the Care Management process to patients/families and other health care team members.
Identifies problematic discharges where there may be issues or obstacles which impact or prolong LOS. Attends unit based problematic discharge meeting. Identifies potential risk and intervenes with system inefficiencies. Collects data as indicated to support recommended changes.

Required Bachelor's Degree Graduate of an accredited school of nursing.
Preferred Will consider non-BSN RN if actively pursuing a bachelor's degree in nursing with completion within 2 years of hire.
2 years of relevant experience Minimum two years' experience in the acute care setting. Required
3 years of relevant experience Three to five years' experience in care management, utilization review, home care and/or discharge planning. Preferred
LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
CRT-Basic Life Support (BLS) - AHA American Heart Association required
