Position Specific - Duties and Responsibilities
Job Specific: there are two levels of practice. Fully licensed Master Level staff provide therapeutic, evidence-based practices of treatment specific to the students need, based on a thorough psycho-social assessment. Bachelor Level Social Workers in a Master’s Social Work program practice within their scope of practice providing support, evidence-based interventions in prevention and intervention under the direct supervision of a master level social worker, fully licensed. Master Level Clinician with Limited or full licensure or limited license in field of practice.
· Current Master Social Work student, with a current LBSW.
· Provide psychoeducation services to aid students and parent/caregivers in understanding student diagnosis, behavioral health needs, and support services offered.
Job duties for both Master and Bachelor level Social Workers:
· Assure correct development and implementation of treatment and prevention plans using best practice models.
· Will provide direct mental health and support services to students.
· Administer behavioral health screening tool.
· Clinical documentation in the electronic record includes amount, scope and duration of services provided per the treatment plan.
· Follows agency policy, DCH, CARF, and Medicaid criteria for clinical documentation for all aspects of the medical record including, but not limited to, intake assessments, person-centered plans, person-centered plan reviews, releases of information, progress/activity notes, annual psycho-social assessments, and discharge summaries.
· Ensures excellent teamwork in all aspects of service implementation through regular meetings and consultation with student, teachers, and families.
· Coordinates and facilitates meetings with families and school staff as requested.
- Works collaboratively with all staff and contracted providers.
- Efficiently and effectively communicates information about treatment interventions updating other team members, school staff and families to assure consistency and success for the student.
- Consults with direct supervisor about any concerns or issues that impact students and/or families care or compliance with regulations within CWN or assigned school district.
- Initial and ongoing training, take part in program monitoring and appraisal, reporting and resolving complaints and problems.
- Participate in monthly supervision individually and/or assigned group supervision with other teams.
- Follow all general agency policy and procedures.
- Works collaboratively with community service providers and other community agencies.
- Participates in local collaborative efforts on behalf of the Agency, School and clients as assigned.
- Recommends interventions and/or modifications to a comprehensive person-centered plan assuring they guided by the child and driven by the family system.
- Assure referrals to community providers and inpatient services are made appropriately, that all health care and community resources are provided according to client need.
- Document on time all clinical documentation within the EMR accurately represents the services directed/provided.
- Attentive to use of time, scheduling with students and families, assuring coverage by the team at times schools are open/or in need of added supports.
- Promotes recovery and empowers students and families in their ability to make positive life choices while promoting good mental and physical health.
- Provide individual (according to scope of practice) or small-group counseling to named students in areas such as coping skills, self-esteem, self-regulation, bullying and interpersonal relationships, and other social conflicts.
- Work with classroom teachers and other professional on the development of class-wide interventions, individual interventions, functional behavior assessments and behavior interventions plans.
- Ensures compliance, within the program, with all applicable laws and regulations.
- Participates in data management and reporting.
- Meet regularly with school-based teams to discuss student strengths.
· Maintain exact records, documenting all interactions.
· Up to date training and understanding of current best practices, research and trends regarding.
skill development, ACES, trauma, and behavior.
· Complete assigned responsibilities within agreed upon or established timelines.
· Perform other duties assigned by the supervisor.
· Understand and work collaboratively with policies and procedures of the school district.
· Collaborate with school staff and community partners to organize and systematically deliver.
Ap required/needed mental health consultations, services, and referrals.
· Aid school personnel as possible with crisis intervention.
· Brief interventions for emotional support and regulation.
· Obtaining and integrating information about a student’s behavior and supports necessary for success in school and life.
