Cherry Health is a Federally Qualified Health Center (FQHC) with a mission to improve the health and wellness of individuals by providing comprehensive primary and behavioral health care while encouraging access by those who are underserved. The Social Worker / Behavioral Health Consultant (BHC) is a member of the patient’s care team, providing brief action focused interventions to address a wide range of physical health, mental health, and substance use disorders to clients in a fast-paced setting. Primary functions of the BHC include identifying and treating co-occurring physical and behavioral health needs in the medical setting: screening, assessments, diagnostic interviewing, coping skills development, and care coordination with internal and external resources, to improve outcomes and decrease barriers for patients accessing healthcare services. This role is integrated into the Barry Community Health Center in Hastings, Mi. This is a full-time, benefit-eligible position working Monday through Friday during the hours of 8 a.m. to 5 p.m.

Requirements:
- LLMSW for a level I.
- Either a Master of Social Work (LMSW) license, or a Licensed Professional Counselor (LPC), by the State of Michigan required – as determined by program requirements for a level II.

- Two years of previous mental health and/or substance abuse assessment and treatment experience preferred, or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job listed below.
- Bilingual/Spanish preferred
Responsibilities:
- Provide brief interventions in an integrated team setting assessing clients’ behavioral health and substance use disorder needs and referrals to treatment.
- Assess and diagnose psychopathology including mental health and/or substance use disorders according to the current Diagnostic and Statistical Manual (DSM).
- Utilize Motivational Interviewing, Brief Action Planning, and CBT (Cognitive Behavioral Therapy) methods to address co-occurring mental health, substance use, and physical health care needs.
- Make referrals internally and externally as needed based on client needs, including coordinating care between providers and therapists.
- Coordinate with community, referral, and funding sources to provide needed services.
- Participate in regularly-scheduled huddles and case conferences with the treatment team.
- Maintain current authorizations for service and complete utilization reviews.
- Coordinate with primary care providers and psychiatrists to effectively treat the biological aspects of behavioral health disorders.
- Provide interventions, safety planning, and/or referrals for patients who are in crisis situations.
- Perform screenings, brief interventions, and referrals to treatment.
- Adequately prepare for and attend required supervision meetings.
- Complete all required training programs and share knowledge and skills with peers.
- Perform other duties as assigned.
