Provides direct social work intervention and cooperative interdisciplinary coordination to/for patients/families in their homes under supervision of LMSW.
Responsibilities:
Provides professional social services to patients and families in the hospice program through the provision of psychosocial assessment, and intervention.
Collaborates with RN on new hospice patients, participating in admit visit when able.
Conducts / completes comprehensive psychosocial assessment of patient/ family within 5 days as part of the comprehensive plan of care.
Participates in the planning and implementation of interdisciplinary plan of care focusing on psychosocial needs of hospice patients/family.
Anticipates and makes appropriate referrals for community resources including financial. Assists with completion of health, financial, and other resource forms as needed and requested by patient/family.
Maintains appropriate documentation concerning all interventions, services and support of the patient/family.
Counsels with patient, family and community as needed to provide support, interventions.
Serves as a resource to hospice staff to asset with stress management.
Conducts bereavement activities, and may supervise bereavement staff and volunteers.
Contributes to effective patient relations by assisting patients, families, co-workers and other health care members to resolve expressed concerns and by demonstrating a positive and helpful attitude.
Assists in SW after hours on call or is available for after hour/weekend psychosocial crisis.
Performs other duties as required.
Required:
Preferred:
- One year experience in hospital, home care, or geriatric social work services.
