What you’ll be doing:
- Utilize the nursing process by delivering patient care, ensuring continuity of care from admission through discharge, and returning to family and community.
- Perform the clinical role of patient educator, caregiver, counselor, consultant, and patient advocate.
- Observe, evaluate and report (when necessary) each resident’s physical and emotional status. Notify the charge nurse, physician, and/or the DON when there is a change in the resident’s condition.
- Identify resident problems and emergency situations and initiate immediate “life-saving” measures in the absence of a physician.
- Provide direct nursing care and provide clinical supervision of patient care staff working with residents assigned on your unit to provide direct care.
- Administer and document medication and treatments and accurately record all care provided.
- Document resident care provided and resident’s response or lack of response to care provided.
- Work with a high quality, collaborative team.

What We're Looking For:
Must be 18 years of age or older.
Must be able to read, write, speak and understand the English language.
Possess an LPN Degree or diploma from an accredited college or university
Must have and maintain a current, valid LPN state license and CPR certification
Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care
Must be able to successfully pass physical examination and background check.
Must also meet state requirements for relevant licensures or certifications and have no disciplinary action in effect against current certification.
History of LCMCF
Suncrest is a 202 bed skilled nursing facility serving those residents who need long-term care and extensive rehabilitation. Lapeer County Medical Care Facility features a 32 bed secure Dementia Unit with special programming for the Alzheimer’s residents. Also, on the facility grounds, is a State of Michigan license child care learning center, licensed for 49 children ranging from two months to six years old. This child care center serves not only employee’s children, but is also open to the public.
