The LPN provides a skilled level of care to patients by assessing physical condition, supporting and assisting with teaching, medication management, and providing personal care to increase the level of comfort, as initiated and under the direction of the RN case manager. The LPN will have advanced skills/certification in medication administration.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Makes skilled nursing visits to patient residence as scheduled by CM RN and manager. Provides all aspects of patient care as directed by the RN in patient's individualized care plan.
Evaluates the effectiveness of treatments and medications, provides and reinforces patient/family education.
Reports on daily visits, findings and interventions on each patient to their Case Manager RN.
Performs patient care with the general assistance and supervision of the case manager according to company polices.
Completes treatments and procedures within scope of practice and acceptable limits and in accordance with the plan of care.
Accurately documents care in the patient's clinical record in a timely manner per McLaren policy.
Actively seeks professional development.
Participates in case conferences or IDT meetings as directed.
Attends staff meetings and all mandatory in- services as established by company policy.
Adheres to ethical nursing standards. Maintains confidentiality and privacy of patient family records, finances, and health status.
Promotes customer service and patient satisfaction.
Participates in rotation of afterhours nursing on call services with RN back up.
Adheres to infection control policies and procedures.
QUALIFICATIONS REQUIRED
Graduate from an approved practical nursing program.
Current Michigan license as an LPN.
Must pass medication examination by 80%25%20or better.
Must have a valid state of Michigan driver's license and provide proof of insurability.
QUALIFICATIONS PREFERRED
- Two years of clinical experience preferred.
