Working under the supervision of Leelin Health Care Inc. qualified nurse the HHA provides supportive services to clients. The HHA will be accountable for providing personal care and provide a clean and save environment for the client, in order to maintain the client's physical and emotional well-being and enable the client to remain at home. On assigned cases, the HHA works under the supervision of a qualified nurse, staffing coordinator and administrative supervisors. Housekeeping duties are a part of every HHA's responsibility.
Essential Functions:
- Encourage the client to become as independent as possible within medically determined limitations and according to the nursing care plan.
- Provide and document personal care such as bed/partial bath, tub bath or shower, hair care, caring of teeth or dentures, helping client in/out of bed, bed making, follows written assignment. Obtain accurate and timely vital signs.
- Help client achieve maximum self-reliance.
- Follow nursing care plan by carrying out and recording specific procedures such as giving cleansing enema, changing non-sterile dressings, emptying catheter and colostomy drainage bags, measure and record intake and output, measure weight gain or loss.
- Follow Universal Precautions.
- Assists clients with prescribed exercises which client and HHA have been taught by appropriate Leelin Health care personnel, including general Range of Motion.
- Assist client to move from bed to chair and to ambulate, if needed.
- Provide 100% of transfer of client if necessary using good body mechanics. May utilize Hoyer lift if available and have been taught by appropriate Leelin health care personnel.
- Assists clients to bathroom or using bedpan.
- Document care provided correctly using telephony on the Generations App.
- Planning, Marketing, and preparing nutritious meals, taking into consideration special diet requirements and client's preferences. Assist client with eating.
- Notify the nurse or staffing coordinator immediately of change in the clients condition which may need office and/or medical intervention.
- Promote client's mental alertness through conversation, reading, games, and other activities.
- Give support and encouragement for the client to strive and achieve goals established by the nurse.
- Do light housekeeping necessary for the healthful, safe, clean and pleasant environment (dishes,laundry,vacuuming,dusting, etc).
- Observe/assist clients with self-administration of oral medications.
- Participate in in-service programs, staff meetings, and other opportunities for learning provided.
- Carry out work according to policies and procedures of Leelin Health Care, Inc. in a manner that reflects appropriate company standards for timeliness, appearance and behavior.
- Participates in client case conference.
- Accompanies client outside of home to medical and social appointments.
- Use appropriately the emergency on-call system after hours.
- Must appropriately follow schedule(s) issued monthly by Leelin Home Health Care, Inc.
Qualifications:
- Preference is given to applicants with successful work experience as home health aides.
- Must be able to pass the Agency HHA written test with a score of at least 80% and correctly perform all activities included in the Agencies HHA competency test.
- Able to deal effectively with the demands of the job including lifting clients.
- Must have an empathetic attitude toward the sick, elderly, handicapped, terminal, dependent, or disabled client in the home setting.
- Able to perform with minimal supervision.
- Preferably one year experience as an aide or a certificate from a recognized aide program or is currently enrolled as a nursing student who has had a minimum of six months clinical experience.
- Have a valid driver's license and dependable transportation to and from work sites. Must make sure the office has a current copy of automobile insurance, if required to transport client in a personal vehicle.
- Must have a working telephone. Any address or telephone number change MUST BE REPORTED TO THE CORPORATE OFFICE IMMEDIATELY.
- Regular and reliable attendance is an essential function of this job.
- Physical requirements: Health status appropriate to the physical and emotional demands of the job.

Supervision:
- HHA is supervised by appropriate nurse every 6 months worked. The nurse will assure the HHA competency in providing care, compliance to NCP/Assignment sheet, appearance of HHA and compliance with the company policy, update or revise the NCP/Assignment sheets during visits.
