Lakeshore Home Health Care Services has immediate openings for Registered Nurses (RNs) for 1st shifts in Muskegon as well as 3rd shifts at Grand Valley State University caring for our pediatric clients in their homes By providing services to htese children with special needs, we are giving the families the time they need to work, spend time with other chilren, run errarnds or get a full nights sleep. For the case at GVSU, we are making it possible for her to live independently and obtain a college degree. This position is under the direction, instruction and the supervision of the Program Manager, for Lakeshore Home Health Care Services, Incorporated (Lakeshore). The Registered Nurse (RN) is a professional medical and health care employee who is specifically trained, and competent to provide the duties of their position. The RN administers skilled nursing care to clients on an intermittent basis in their place of residence. This is performed in accordance with a physician's orders and client plan of care under the direction and supervision of the Program Manager. WHAT WE OFFER* Health insurance * Dental and vision insurance * Paid vacation * Sign-on and referral bonuses QUALIFICATIONS* Must be a graduate of a state approved school of nursing, and currently possess a valid RN license in the State of Michigan. * Must not have a criminal conviction for any violent crime or felony, and no past or pending disciplinary actions as an RN. * One (1) years experience in nursing preferred. * The ability to exercise initiative and independent judgment. Responsibilities include but are not limited to:* Performs ongoing assessments of client's status and regularly reevaluates the patient's nursing needs to modify care plan as necessary. * Asses the physical, psycohoscial, spiritual and environmental status of assigned clients in a systematic and continuous manner to provide required services. * Consults with physicians, other staff, client/family and other caregivers to formulate a plan of care and coordinate delivery of services to ensure high quality of care. * Initiates care and performs services in accordance with the clients' plan of care. * Continuously evaluates the clients status and works with the client/caregiver tin developing ways to meet their needs. * Informs the physician and other healthcare team members of changes in the client's condition and needs. * Prepares clinical and progress notes for each client visit and summaries of care conferences on his / her client in a timely manner as directed and per Lakeshore policy. * Coordinates client services as directed by the Program Manager. * Counsels and educates the client and the clients family as appropriate to meet identified needs and achieve goals gives medications according to agency policy * Adheres to standard precautions. * Participates in and presents in-service programs. * Processes orders and notifies physician of client needs and changes in the clients condition, Lakeshore certification / recertification orders and discharge summaries. * Supervises and teaches other skilled nursing personnel as appropriate. * Conducts client care conferences on client(s) assigned to their care. * Informs clients of their rights and responsibilities and respects these rights and responsibilities * Complies with all agency policies and procedures participates in Performance/Quality Improvement activities as indicated. * Participates in committees as requested. * Maintains an accurate account of medical supplies in his/her possession.
