Provides peer support and role modeling related to the improvement of life skills and health/wellness to dually eligible Medicare and Medicaid members during personal interactions including home visits, telephone calls, etc. Instructs members about wellness, chronic disease prevention and self-management. Assists members with accessing appropriate community resources. Assists with the coordination of group support programs and provides ongoing contact. Cultural competency and shared-life experiences with this population is preferred. Must have reliable transportation and be able to drive to and from appointments. This position will be working with our Dual Special Need Plan (Medicare and Medicaid dual enrolled) members in the Kalamazoo and surrounding areas.
Education - High School Diploma or equivalent
Successful completion of the paraprofessional CHW Training Program (post-hire into role) 180 Days
Drivers License – State of Michigan
Proof of valid automobile insurance (submitted at each renewal)
Education - Post high school education
Experience - 3 years of experience typically gained through skills/knowledge/abilities in Work experience Knowledge and use of community resources; social, health and public agencies and services
