Provides quality education, counsel, and support to clients and their significant others regarding diabetes self-care. Documents the education process, patient information and clinical recommendations according to department/program/regulatory guidelines, and measures outcomes to maximize continuity of care between settings. Participates in projects related to area of specialty and the development/revision of procedures/policies, educational materials/guidelines, educational programs. Collaborates with and serves as a resource for other health professionals to promote delivery of quality services and patient education within the institution as well as the community.
Education - Bachelor's Degree in Nursing or related healthcare field
Experience - 3 years of experience typically gained through skills/knowledge/abilities in recent, relevant clinical experience including prior teaching experience with adult clientele
Licenses - Registered Nurse (RN)
Dietitian – Registered (REG DIET)
Cardiac Pulmonary Resuscitation (CPR) within 90 Days of Hire
Diabetic Educator – Certified (CDE) Once eligible
Experience - Working with Diabetic speciality as an Educator and/or Nurse
