GENERAL SUMMARY:
Under direction and guidance from a fully-licensed MSW Social Worker, the BSW Social Worker, provides psychosocial assessments, discharge planning and other social work services for patients and their families with personal and/or environmental difficulties which predispose to illness or interfere with obtaining maximum benefits from medical care.
A member of the Mental Health EC Intake Team, provide social work services to psychiatric patients under the supervision of the Manager of Social Work
ESSENTIAL DUTIES:
Assesses the psychosocial needs of patient and address needs of family members that are relevant to optimizing the patient's care management process.
Works along with the Home Care team to facilitate appropriate placements in nursing homes, hospice, etc.,
Directs patients and their family members to community agencies for assistance related to identified needs.
Provides counseling pertinent to patient/family.
Working closely with and under the direction of a fully-licensed MSW Social Worker, the BSW Social Worker promotes the welfare of others through the initiation of child and adult protective services referrals
Accurately completes documentation within the time frame of agency policy.
Serve as a primary resource for coordination of care from EC to next level of care in Mental Health (MH) system.
Monitor LOS, complete audits, speak to EC Dr.'s and/or psychiatrists.
Communicate with 3rd party papers/utilize management to approve admissions to next level of care
Provide concise documentation re: approved admissions; authorization number, representative for 3rd party payor.
The ability to prioritize in a high stress environment.
Conducts and documents psychosocial evaluation in the EC.
Maintain communication with sister hospitals and other agencies to determine next level of care for patient.
Attends and participates in quality meetings to improve processes.
Team work oriented.
Provide discharge planning follow up for patients DC form EC.
Understands petition/cert/legal process/recipient rights to mental health patients.
Other duties assigned.

Education / Training:
Bachelors' Degree in Social Work (or equivalent), LBSW: LLMSW
Work Experience:
At least 2-5 years of Social Work experience in a health care setting.
Certification, Licensure, Registration:
State of Michigan Social Work License.
Other Qualifications:
Excellent interpersonal and communication skills, oral and written.
