Position Summary
This position provides care to participants aged 55 and older in an adult day center setting following the regulations and guidelines established for PACE organizations.
Primary Responsibilities
Provide personal care, homemaking, and respite support in home environment, and in day center environment as needed.
Regular collaboration with other members of the interdisciplinary team to ensure patients' care needs are met.
Assists all participants with activities of daily living (ADL), as needed according to their plan of care.
Document in the participant's medical record as required by policy.
Skills and Knowledge
Able to read, write, speak, and understand the English language.
Possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
Function independently and have flexibility, personal integrity, and the ability to work effectively with older adults, personnel, and support organizations
Must possess excellent communication skills with individuals of all ages, especially the geriatric population
Must be organized and able to complete tasks efficiently
Must possess strong sense of professionalism, good critical thinking, creativity, flexibility, and excellent problem-solving skills
Must possess high ethical standards
Must work well on a team
Proficient in Excel, Microsoft Office, Outlook, and electronic medical records

Required Qualifications
Current Certified Nurse Aide certification in the State of Michigan from an accredited program
Must have reliable transportation and valid drivers license
Must have current CPR certification (or willing to obtain)
Desired Qualifications
One year of experience working with the frail elderly, long term care population, or end-of-life care.
