We are currently accepting applications for a part-time Care Partner with second shift availability (3:00pm-11:00pm).
All positions require holiday and weekend rotation shifts.

POSITION SUMMARY
In the Elder-centered environment of the Meadowlark Household, life is organized around the needs, preferences and lives of the Elders who live there. The constant companion of the Elder is the Care Partner. The Care Partner’s primary responsibility is to ensure the high quality, person-centered care of the Elder and to protect the Elder from the three plagues of loneliness, helplessness and boredom.
The work of the Care Partner is organized around a self-organized work team. Team members work closely together to assist one another in supporting the Elders and achieving the goals of the Household. Members of the self-organized work team serve in various coordinator roles to facilitate the delivery of services and compliance with regulations and operational and service plan goals.
The Care Partners are coached by the House Manager/Coach/Mentor, who oversees operations and assures compliance with all regulatory and organizational requirements in partnership with the Quality Guide. The Care Partners work cooperatively and simultaneously with the Nurses and other clinical staff to support their work.
Job Description
The Care Partner will provide care to the Elders living in the Meadowlark Household. The primary responsibility of the Care Partner is to protect the Elders from the three plagues of loneliness, helplessness and boredom by being in relationship with the Elder, providing assistance with activities of daily living and meeting other needs as required, in a manner that promotes a positive, high quality life.
The specific responsibilities of the Care Partner include the provision of high-quality care, the maintenance of a safe and clean environment, food preparation, cooking, serving and cleanup of all meals and snacks, laundry, and housekeeping. In addition to these tasks, the Care Partner is in relationship with the Elder, the Elders’ family and their co-workers and promotes community within the Household. They actively support Elder choice and the meaningful and mindful engagement of those in their care.
The Care Partner is a member of a consistently assigned, self-organized work team and is responsible for interacting with all team members in a manner that is respectful and collaborative. Each self-organized work team member is responsible for coordinating aspects of the team’s work.

- High School Diploma or GED
