With the guidance of a fully licensed Master Level Therapist, the Case Manager will gather information about clients using case histories, observations, and assessments, which will aid in the development of a treatment plan as well as the aftercare plan best suited for the client. The Case Manager will also be responsible for facilitating and leading groups and didactic sessions as well as managing a caseload and completing required documentation.
Role and Responsibilities
- Designs and implements strategies for obtaining services and supports that are goal-oriented and individualized.
- Provides assessment, planning, linkage, advocacy, coordination, and monitoring to assist persons served in gaining access to needed health and dental services, financial assistance, housing, employment, education, social services, and other services and natural supports developed through the person-centered plan process.
- Must be effective, efficient, responsive, and organized while focusing on process and outcomes.
- Establishes and maintains constructive and cooperative working relationships with those we serve and co-workers.
- Observes, receives, and otherwise obtains information from all relevant sources to serve assigned persons.
- In partnership with the person served, develops an IPOS that complies with defined service specifications and the Mental Health Code.
- Develops and communicates expectations and negotiates to ensure that the provider of services (including primary health) has all relevant individual information to implement the plan and provide effective services successfully.
- Monitors and identifies health changes according to the individual’s plan and take appropriate action, as needed.
- Effectively problem-solves and resolves conflict to ensure that the individual maintains control of decisions for their life.
- Effectively relates and interacts with individuals who are experiencing symptoms of a severe mental illness, with or without co-occurring substance use disorder, and understands how behaviors are sometimes used to communicate feelings and/or undiagnosed medical conditions or pain.
- Participates in projects utilizing the project management framework as assigned.
- Assists patients with the completion of short-term disability and FMLA paperwork.
- Ensures completion of data collection surveys and exit surveys.
- Complies with organizational policies and procedures, including code of conduct; and policies resulting from the HIPAA privacy rules regarding PHI concerning persons serviced or employed.
- Works on special projects and supports coordination/case management activities, as assigned.
- Performs other duties as assigned.
Qualifications and Education Requirements
- Bachelor’s degree in a human services-related field (i.e., Social Work, Psychology, Sociology), is required.
- Licensed BSW, preferred.
- Qualified Mental Health Professional (QMHP), preferred.
- Possession of a valid driver’s license.
- Two (2) years prior Case Manager experience, preferred.
Knowledge/Skills/Abilities
- Strong organizational skills, with experience in person-centered planning.
- Demonstrated understanding of persons with several mental illness and/or substance use disorders.
- Demonstrated communication skills that effectively convey information.
- Demonstrated active listening (i.e., listening to what other people say and asking appropriate questions).
- Knowledge of trauma-informed theories, principles, and practices (includes a multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions).
- Demonstrated leadership skills and good decision-making ability.
- Knowledge of case management.
- Ability to work with a team milieu approach to the treatment of clients.
- Knowledge of community resources.
- Knowledge of counseling theories and interpersonal dynamics.
- Ability to identify own strengths and areas of improvement and communicate needs to supervisor.
- Applied knowledge of de-escalation techniques.
- Ability to converse verbally and in writing with individuals served, physicians, providers, co-workers, and employees regarding services.
- Ability to travel independently to various facilities, programs, and assigned work locations.
- Ability to independently operate a personal computer.
Physical Requirements
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift 15 lbs.
- Must be able to perform repetitive tasks such as typing for extended periods of time.
- Must be able to clearly express and exchange ideas by means of spoken word to impart oral information to employees or others accurately or quickly, engaging in sometimes lengthy conversations or presentations.
- Must be able to ascend and descend stairs daily.
- Must be able to seize, hold, grasp, turn or otherwise work with your hand or hands.
- Must be able to perceive sound by ear to interpret oral information from employees or others accurately or quickly.
- Work will be performed in an office environment with fluctuating temperatures.
