Position Summary
Seeking a Board Certified/Board Eligible Physician to join our innovative Hospice & Palliative Care team.
Position Highlights:
Robust, interdisciplinary inpatient team providing palliative consultation throughout the health system in the Grand Rapids area
Inpatient Palliative Care consults provided on the oncology, cardiac and intensive care units, the acute care unit for the elderly, in the ED department and on the Supportive Care unit for acutely symptomatic patients on hospice care
Outpatient Palliative Care clinics for oncology and heart failure patients including telemedicine
High functioning, energetic interdisciplinary team including physicians, mid-levels, nursing, social work and chaplains
Advanced expertise in pain and symptom management with a holistic, team approach Integration with a national Palliative Care database providing leading quality metrics, benchmarks, expert level forums and educational activities
Excellence in patient directed care including goals of care discussions and advance care planning
Team of board certified physicians and a highly experienced advance practice providers
Collaboration with integrated Post-Acute Service Lines including home palliative care, home based primary care services, the Medicare Care Choices program and one of the largest hospice programs in the state
Partnership with several educational Institutions in the areas, providing opportunity for work with medical students, residents, and fellows
Research involvement as we develop best practice in symptom management, patient directed care and services that address social determinants of health
Aggressive plans for continued growth to meet the needs of the community and within the broader West Michigan area

Education - Medical Degree (MD or DO) in Medicine
Licenses -
Current M.D. or D.O. license for the state of Michigan.
Current State of Michigan and Federal Narcotics Licenses.
Basic Life Support (BLS) Specific to SHMG 120 Days
Advanced Cardiac Life Support (ACLS) Specific to SHMG: ACLS required if stress test is performed or as required by specialty. 120 Days
