GENERAL SUMMARY:
Under the direction of the program leadership of social service, the social worker interviews patients and family members to obtain an age-specific psychosocial assessment and assists patients and families in adjustment to illness and disabilities and resolving personal financial and environmental difficulties, which interferes with the care management process. Provides crisis intervention, individual and family therapy where skilled social work judgment is required. Direct access to appropriate community and adjunct resource that foster improved quality of life. This includes counseling and seeking resources to answer unmet needs. The clinical social worker also provides consultation services and in-service education programs related to the psychosocial aspects of health care to other professional staff members on the healthcare team.
ESSENTIAL DUTIES:
Assesses and diagnoses the psychosocial needs of the patients, caregivers, and families--how these needs are impeding the plan of care, as evidenced by documentation on the plan of care and clinical narrative within 24 hours of psychosocial assessment to determine a client's needs.
Upon assessing patient needs, social worker makes all appropriate referrals to other community service agencies for assistance in a timely manner, as evidenced by documentation.
During each visit, the plan of care, discharge plan and outcome criteria are reassessed to meet client and family/caregiver needs and ensure quality of care, as evidenced by documentation in the clinical record.
During each visit demonstrates the skills and judgment necessary to implement the plan of care, interventions, and procedures necessary for the care of the client, as evidenced by the chart review and direct supervision.
During each visit performs all duties/responsibilities required by the home and community services division to ensure that the unique nature of the client is addressed. This includes the age, personality, and psychosocial needs of the client served, as evidenced by documentation in the clinical record and direct supervision.
Is responsible for utilizing clinical supervision to better meet the psychosocial needs of the patient and family to develop improved clinical skills and expertise, as demonstrated in weekly/biweekly meetings with the director of social services/program manager.
Provides consultation to other professional health personnel regarding psychosocial issues and in-service education to agency staff, as evidenced by case conference notes, grand round notes, and documentation in clinical charts.
Assesses the community environmental resources available to facilitate long and short term plans for post discharge planning in collaboration the case manager.
Maintains a relationship with other mental health/social service agencies in the area, as needed and evidenced by documentation in the clinical record and discussed in supervision with the program manager.
Works to attain an increasing knowledge of counseling and therapy practice by attendance at pertinent in-services and seminars, as evidenced by education logs and CEU certificates.
Assumes responsibility for welfare of others through the initiation of child abuse referrals, adult protective service referrals, guardianship petitions and psychiatric petitions. Performs the assessment, initiates referral and appear in court as subpoenaed by the court system.
Attends all required safety training programs and can describe his or her responsibilities related to general safety, department/service safety, and specific job related hazards.
Follows the home and community services division exposure control plans/blood borne and airborne pathogens.
Promotes effective working relations and works effectively as part of a department/unit team inter- and intradepartmentally to facilitate the department's/unit's ability to meet its goals and objectives.
STANDARD QUALIFICATIONS
*Education / Training:
*A master's degree in social casework from an accredited graduate school of social work, as defined by the council on social work education.
*Work Experience:
*Three years of social work and medical geriatric/home-care experience.
*Crisis intervention, brief solution-focused therapy, and knowledge of community resources required.
*Managed-care experiences a plus.
*Certification, Licensure, Registration:
*Licensed in the state as an LICSW (licensed independent clinical social worker). If working under a limited license, full licensure must be obtained within 6 years as required by the State of Michigan.
