We have an exciting opportunity for a Case Manager- Children’s Outpatient (COP) to join our team of talented professionals.
To qualify for this position, you will need:
- Bachelor’s degree in social work, psychology, or mental health-related field from an accredited school.
- Candidates must obtain state licensure (LLBSW, LBSW, LLMSW, LLPC, TLLP, LMSW, LPC or LLP) within a 60-day training period beginning on the date of hire. Failure to obtain licensure within the training period will result in the employee being unqualified to perform work functions. Beyond the training period, all state licensure must continually remain active, in effect, and free from disciplinary action
- Must also meet Michigan Department of Community Health requirements for a Child Mental Health Professional (CMHP) within 120 days of hire.
- Course work, fieldwork or job experience will demonstrate competence in working in a collaborative way with clients and community, cultural competence, good organizational skills, and the ability to work both independently and as a team member.
- 3 years supervised experience in the examination, evaluation and treatment of minors and their families or a master's degree and 1 year experience in the examination, evaluation and treatment of minors and their families is required.
- Valid chauffeur’s license and favorable driving record is required. Candidates who do not have a chauffeur’s license may still apply, but must be able to obtain one within 30 days of hire.
- Basic computer skills.
- Additional requirements include: cultural competency, ability to work effectively with a diverse population, the ability to engage and maintain therapeutic relationships with children and families in a variety of modes of treatment, the ability to develop collaborative relationships with other unit staff and providers, effective communication skills, and a willingness to be flexible.
Position Summary
The COP Case Manager is responsible to provide assessment, linking, coordinating and monitoring, family education and support, transportation, treatment planning and follow-up services to children and families. Services are delivered in the office and in the community based on the needs of the child, family. The Case Manager is responsible to help coordinate and integrate services that are provided to the family both internally and externally. Regular working hours are Monday through Friday 8:30 – 5:00 with 4 hours after 5 p.m. per week required. In addition, due to program and consumer needs weekends may be required.
