Position Summary
Our home health agency is an accredited agency that is hiring a home health registered nurse that is seeking a patient-centered care model of nursing. We work with patients of all ages to provide safe, quality care in a patient’s home within a 70 mile radius of Grand Rapids, MI. Our average case load of patients per registered nurse is 20 patients. Registered nurses use their clinical judgment and assessments to coordinate the care of the patient with an interdisciplinary team.
Responsibilities:
- Assess and educate patients each visit and document at point of care;
- Perform evaluation tasks, including vitals and med review;
- Administer and educate patient or caregiver on medications ordered by physician;
- Wound care;
- Coordinate care with all disciplines;
- Ability to travel to multiple patients per day.
*** Bonus is up to $5,000 eligible to external candidates ***Basic Qualifications:
Education - Associate's Degree or equivalent in Nursing
Experience - 1 year of experience typically gained through skills/knowledge/abilities of acute care, emergency, or rehab facility experience
Licenses - Registered Nurse in the State of Michigan
Current State of Michigan Driver's License and car insurance - Upon hire
Basic Life Support within 90 days
Preferred Qualifications:
Experience - 2+ years homecare or med surg experience
