About the Role
As the RN Care Manager, you are a part of a revolutionary service coordinating all patient care between primary, specialty, inpatient and behavioral health services. You serve a variety of patients including individuals with substance use disorders and both children and adults with mental illness.
Your role is to assist clients in initial and ongoing care coordination including:
- Monitoring client’s status with ongoing assessment and screenings.
- Collaborating with the client to develop an initial care plan.
- Identifying strategies to implement evidenced-based wellness and prevention initiatives.
- Communicating with medical providers, substance abuse service providers, long term care and hospitals.
- Providing education on health conditions, treatment recommendations and medication.
