Job Details

Summary:

Under the direction of the NS, and working with Operations Manager (OM), Residential Supervisor (RS), Care Coordinator (CC), and Living Skills Staff (LSS), the PCT primarily attends to medication administration and medication systems of the residential programs.

            Essential Duties:

1. Model appropriate behavior conducive to a positive working and therapeutic environment.
2. Observe all rules of confidentiality as they pertain to program participants internally, and in dealing with outside individuals and/or agencies.
3. Proactively initiate and follow-through with tasks utilizing appropriate problem solving techniques in order to facilitate communication flow and prompt resolutions.
4. Assume responsibility for knowledge and implementation of program policies and procedures.
5. Attend staff meetings, in-services, etc. as necessary and as assigned.
6. Assist and/or provide intervention relating to activities of daily living (ADLs), medications, wound care, bowel/bladder, peg tubes, catheters, diet/nutrition, nebulizers, CPAPs, enema/suppository, vitals, weight, etc. as directed by the NS. 
7. Actively involved with infection control, including but not limited to facility sterilization, cleaning, laundry, etc.
8. Report client issues relating to medical needs including medications (i.e. refusals), medication changes, ADLs, wound care, etc. to the NS and appropriate team members.
9. Communicate effectively with the NS and team members to clarify and resolve issues involving obtaining, storing, delivery, inventorying, and administrating medications.
10. Implements policies and procedures defining Origami’s medication system in conjunction with the NS. 
11. Maintains working knowledge of information about the medications delivered and medication system.
12. Ensures cleanliness and organization of content and maintenance of the medication room and supplies.
13. Under the direction of NS disposes unused and extra medications according to procedure.
14. Assures that medications are kept in a safe and satisfactory state according to Origami policy and adult foster care state licensing regulations.
15. On-call PCTs must be available to work a minimum of three shifts per month as to keep current on procedures and care plans.

            As Needed:

1. Increase employee awareness of educational resources about medications.
2. Adds to and updates onsite availability of resources regarding medications and medical issues.
3. Available for on-call emergencies as necessary.
4. Responsible for finding or providing coverage in emergency situations during designated times.
5. Understand and follow emergency procedures.
6. Fill in for team members during absences as designated (including LSS when necessary).
7. Report equipment and building repair.
8. Complete accidents, occurrences, and/or medication incident reports according to policy.
9. Other assigned duties.

 Communication:

1. NS /LSS – in order to clarify client’s treatment and identify potential problems
2. Team Members – in order to facilitate client’s treatment and discuss procedure.
3. External Stakeholders – in order to provide education and updates related in individual treatment plans.
