Clinical Case Manager
The Clinical Case Manager provides comprehensive, individualized Targeted Case Management (TCM) services to adults with Serious Mental Illness (SMI), Intellectual/Developmental Disabilities (I/DD), and co-occurring substance use disorders.
This position coordinates care, advocates for individuals, monitors service needs, and collaborates with multidisciplinary teams to promote health, safety, independence, and positive outcomes. All services and documentation must comply with the Michigan Mental Health Code, MDHHS Medicaid Provider Manual, Community Mental Health (CMH) standards, and organizational requirements.
Essential Responsibilities
Assessment & Care Planning
- Conduct initial and ongoing clinical assessments identifying individual strengths, needs, preferences, barriers, and health/welfare risks.
- Complete required assessments, including intake/re-assessment, LOCUS, and biopsychosocial assessments as applicable.
- Facilitate the Person-Centered Planning process and development of the Individual Plan of Service (IPOS).
- Develop measurable, individualized goals and identify the appropriate frequency, scope, and intensity of case management services.
Resource Coordination & Advocacy
- Link individuals to needed medical, dental, behavioral health, financial, employment, education, housing, and community resources.
- Advocate for individuals when navigating complex systems, including DHHS, schools, healthcare providers, and hospital networks.
- Coordinate with primary care providers, psychiatric providers, therapists, and other members of the treatment team.
- Promote integration of physical health, behavioral health, and community-based supports.
Monitoring & Service Review
- Conduct regular face-to-face monitoring contacts based on individual clinical needs.
- Monitor IPOS implementation and assess whether services are meeting identified goals and needs.
- Identify changes in condition, safety, health, or circumstances and coordinate appropriate modifications to services.
- Proactively identify and address gaps or barriers in service delivery.
Documentation, Compliance & Billing
- Maintain accurate, timely, and audit-ready electronic clinical records.
- Document required contact details, including date, start/end times, duration, location, nature of service, and whether contact was face-to-face.
- Ensure services meet applicable Medicaid, CMH, Michigan Mental Health Code, and organizational requirements.
- Maintain appropriate professional boundaries and ensure case management activities remain within authorized scope of practice and regulatory requirements.
- Complete documentation and other assigned responsibilities within required timelines.
Team Collaboration
- Participate actively in multidisciplinary treatment planning and case review.
- Communicate effectively with individuals, families, providers, community partners, and internal teams.
- Collaborate with other departments and complete additional duties as assigned.
Population & Caseload
The position serves adults with varying levels of need, including:
- High-acuity/dual-diagnosis individuals, including I/DD with severe mental illness and/or high-risk ASD.
- Adults with I/DD and moderate ASD.
Typical caseload: 15–25 individuals.
Education & Qualifications
- Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or a closely related human services field from an accredited institution.
- Minimum 1–2 years of specialized, direct experience working with individuals with SMI, SED, and/or I/DD.
- Active Michigan professional licensure, such as LLMSW/LMSW, LLPC/LPC, or LLP/LP, as applicable.
- Relevant case management and clinical coordination experience preferred.
- Strong communication, organization, documentation, and problem-solving skills.
- Ability to work effectively within a multidisciplinary team.
- Proficiency with electronic health records and care management software.
- Valid driver’s license required.
- Ability to manage multiple priorities while maintaining accuracy, confidentiality, and compliance.
