Physical Therapist – Full-Time | Bronson at Home (Home Care)

What You’ll Do

Complete comprehensive assessments of mobility, strength, balance, coordination, and functional abilities

Develop individualized treatment plans and measurable goals

Provide evidence-based physical therapy interventions in the home setting

Educate patients, families, and caregivers on exercises, safety, and self-management
-
Collaborate with physicians, nurses, and the interdisciplinary care team

Recommend and train patients on assistive devices and adaptive equipment
-
Support quality improvement and serve as a clinical resource within the rehab team
Required Qualifications

Graduate of an accredited Physical Therapy program
-
Current Michigan Physical Therapist license
-
Michigan driver’s license, reliable transportation, and auto insurance
-
BLS certification within three months of hire

Strong communication and interpersonal skills

Basic computer and mobile device proficiency

Key Responsibilities
Clinical Care

Interpret physician referrals and establish functional baselines

Deliver therapeutic activities and exercise programs

Provide patient, family, and community education

Assist with goal setting and treatment duration

Design or recommend assistive devices

Provide coverage across Rehabilitation Services as needed
Documentation & Administration

Complete timely, accurate documentation in the electronic health record

Maintain confidential and compliant patient records

Communicate documentation barriers to leadership

Participate in committees, task forces, and process improvement

Demonstrate strong organization and time-management skills
Physical Requirements
This role requires frequent standing, walking, bending, and assisting with patient mobility. Candidates must be able to lift and move light objects and safely support patient transfers and positioning.
