Starting Salary: $70,000 - $85,000/yr commensurate with experience

What You Will Be Doing
As a Nurse Care Manager, you will have the opportunity to specialize in integrative health by coordinating patient care across the behavioral and physical health spectrum of services. You will be responsible for a caseload of at-risk patients and will work as a liaison between ISK and other physical health providers to ensure your patients receive appropriate services and continuity of care. This may include triaging and coordinating referrals and follow-up, connecting to resources and/or programs, and assisting with metric compliances for CCBHC. You will develop an integrated health care plan for your patients and work in collaboration with other providers to ensure care plan goals are met. Additionally, you will be responsible for tracking outcomes of patients assigned to your caseload. This position provides for great work/life balance! Primary hours are Monday - Friday. No holidays or weekends!
What We Require
Associate’s degree in Nursing (Bachelor’s preferred) along with a valid RN license in the State of Michigan, valid driver’s license and reliable transportation. A minimum two years’ experience in the primary care or mental health field, preferably in a community setting. Strong understanding of recovery-oriented services for individuals who have psychiatric needs, are intellectually/developmentally disabled, and/or who have substance use disorders. Intermediate computer skills, including MS Word, Excel, and Outlook. Strong experience with electronic health records and the ability to work within more than one system.

Physical Requirements / Working Conditions:
Must be able to perform essential job functions with or without reasonable accommodations, including, but not limited to, visual and/or audiological appliances and devices to increase mobility.
Physical Efforts – Job demands include prolonged sitting and standing. Requires light lifting up to 25 pounds, stooping, kneeling, crouching, or bending. Requires coordination of hands and/or eye/hand/foot and ability to hear and communicate verbally.
Working Conditions – In office environment with noise from computers, copy machine, and telephones. Use of computer for periods in excess of 30 minutes at a time. May be exposed to bloodborne pathogens, infectious diseases, and parasites. May encounter community environments that include homes, schools, courts, and other community settings that may or may not be barrier-free. Travel throughout the Kalamazoo area may be required.
