Role Summary
The Case Manager is responsible for guiding patients from admission through discharge with seamless, coordinated care. This includes assessment, planning, implementation, monitoring, and evaluation of the patient caseload. The Case Manager ensures care is appropriate to patient needs and the hospital’s capabilities, serves as an advanced clinical resource for patients, families, and staff, and collaborates with the interdisciplinary team to deliver comprehensive, cost-effective care. Continuous improvement and outcome monitoring are key components of this role. Employees providing direct patient care must demonstrate competencies specific to the population served.
Qualifications

BSN required for all new hires beginning March 31, 2014

Master’s degree preferred

Three years of acute care hospital experience required

Licensed Registered Nurse in good standing with the State of Michigan

Case Management Certification preferred

Proficiency with word processing, spreadsheets, presentation software, and other job-related programs

Physical Demands
This role requires sustained concentration and attention, including computer use for 40 to 70 percent of the time. Physical demands include regular walking, standing, stooping, bending, sitting, and occasional lifting of light objects.
Key Responsibilities
Patient Assessment and Care Coordination

Conducts early assessment to identify patients at risk for post-hospitalization needs.

Performs interviews with patients and families and reviews health records and psychosocial factors.

Develops, reviews, and revises transition care plans with the care coordination team.
-
Manages and monitors patient progress and documents according to procedure.
-
Rounds daily on assigned patients and maintains ongoing communication throughout care.
-
Identifies readmissions, reasons for readmission, and interventions to prevent future occurrences.
-
Ensures smooth and efficient transitions of care between services.
-
Leads multidisciplinary team rounds.
-
Documents clear and specific transitional planning aligned with patient needs and preferences.
-
Coordinates post-discharge resources including Medical Social Work, Pastoral Care, rehabilitation, long-term care, home health, and community services.

Acts as a liaison between patients, physicians, ancillary departments, and community services from diagnosis through post-discharge.

Develops and communicates the plan of care from acute phase through transition.

Builds strong relationships with physicians, nursing teams, and leaders.

Coordinates care using Pathways or Plan of Care and contributes to their development and revision.

Participates in committees and assigned groups.
-
Ensures appropriate use of community and outpatient resources after discharge.
-
Manages discharge plans to ensure appropriate length of stay and achievement of quality, clinical, and financial outcomes.
-
Evaluates outcomes related to case management including LOS, readmissions, patient satisfaction, and financial variances.
-
Tracks and trends outlier LOS data to reduce variances.
BLH and BSH Specific Responsibilities
House Manager duties include:

Coordinating and overseeing hospital operations in the absence of Unit Leaders.

Resolving crises and conflicts and informing leaders of issues and resolutions.
-
Investigating complaints from patients, visitors, physicians, and staff and implementing follow-up.
-
Serving as initial contact for atypical events, conducting assessments, initiating action, and notifying appropriate departments.
-
Assessing safety concerns and taking appropriate action.
-
Coordinating admissions, transfers, and patient placement.
-
Monitoring hospital capacity and collaborating with Staffing Office.
-
Conducting regular rounds of all departments and units.
-
Maintaining utilization review and monitoring infection control data.
-
Leading Safety Check-In meetings on weekends and holidays.
-
Answering Employee Incident Hotline calls after hours and documenting incidents.
-
Collaborating with ER physicians for prophylactic treatment in exposure situations.

Participating in After-Care duties when required.
BBC Specific Responsibilities

Collaborate with multidisciplinary teams and providers to create standard care plans and review them with patients as needed.

Assess eligibility of uninsured and underinsured patients for federal, state, and community programs and assist with paperwork.

Partner with Patient Financial Counselors as needed.

Serve as a liaison to community programs, participate in community meetings, and maintain a network of resources to support patient needs.
