RESPONSIBILITIES: This position serves as a member of the Pathways Emergency Services/Access Team. Responsibilities
include, but are not limited to conducting pre-admission screenings, providing crisis intervention and safety planning,
completing access screenings and making appropriate referrals, monitoring court-order treatment, delivering on-site mobile crisis response, coordinating discharge planning, and serving as liaison to emergency departments, inpatient psychiatric units, courts, correctional facilities, and homeless shelters.
This clinician functions as an expert consultant to Pathways providers regarding the development and negotiation of Person-Centered Plans for individuals receiving emergency and intensive services as alternatives to hospitalization. This position’s role involves brief, solution-focused treatment relationships emphasizing safety planning and ensuring a “warm handoff” to appropriate community-based services.
This position must demonstrate comprehensive knowledge of Severity of Illness/Intensity of Service (SI/IS) criteria and maintain familiarity with community support resources across all areas served by Pathways. Strong collaboration skills are essential, as this role requires developing effective working partnerships with colleagues across multiple populations, programs, and locations. Clear, timely communication with community partners, service recipients, and other stakeholders is critical. Due to the acuity and intensity of services provided, the clinician must perform responsibilities accurately and efficiently in fast-paced situations.
Participation in a 24-hour on-call emergency services rotation and travel within the service area are required.
QUALIFICATIONS: Master’s Degree in Social Work, along with State of Michigan licensure as a Masters of Social Work (LMSW) or a Limited Licensed Masters of Social Work (LLMSW). Other Master’s degrees with appropriate licensure may be considered. One year of experience working with persons with mental illness which includes inpatient discharge plan negotiation and / or crisis support services activity preferred.
