GENERAL SUMMARY:
Supervises and coordinates nursing activities of assigned shift to ensure that the ongoing needs of residents are met. Observes, assesses, and documents status and symptoms of residents, provide nursing care to residents and effectively communicates to residents, other staff, families, and physicians.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Oversees the nursing care provided on the unit assuring that the nursing process is utilized.
- Briefs personnel on resident’s condition and appropriate care necessary at the beginning of each shift.
- Demonstrates knowledge of IV therapy and drugs.
- Demonstrates knowledge and practice of standard precautions.
- Demonstrates thorough knowledge of disaster plans and ability to coordinate their execution.
- Ensures completion of transcription of doctor’s orders.
- Obtains consents as needed.
- Supervises the direct care rendered by LPNs and CNAs. Acts as a professional role model and resource.
- Demonstrates knowledge of philosophies, policies, and responsibilities of nursing service.
- Knows and maintains Federal and State Regulations.
- Completes admission, transfers, and discharge of residents.
- Utilizes written and verbal reports to ensure safe continuity of care 24 hours a day.
- Completes nursing assessment of a new resident within 24 hours of admission.
- Demonstrates effective customer service skills on an ongoing basis.
- Makes on-going assessment of residents’ condition, determines nursing diagnosis, develops, implements and updates resident plan of care, initiates and resolves temporary plan of care, and evaluates outcomes and effectiveness.
- Oversees and assists with resident care documentation, per policy and per procedure, including:
- Daily Charting
- Weekly Summaries
- Med Errors
- CNA flow sheets
- Observes, assesses, and documents resident’s conditions and communicates appropriately with family and physicians, unit coordinator, Director of Nursing, and ancillary staff.
- Assists in emotional and physical rehabilitation and education of residents.
- Assists physicians on monthly rounds and reviews medical plans of care with him/her.
- Assures that significant changes of condition are reported to the physician and family/POA, unit coordinator and CNAs in a timely manner.
- Supervises notification of families or responsible party when a resident incident occurs.
- Attends all mandatory in-service programs and attempts to attend all other relative in-service available at the facility keeping abreast of the current nursing practices.
- Assures adequate staff to meet resident’s needs.
- Evaluates unit staff and participates in teaching and guiding staff.
- Uses disciplinary process per policy and procedure maintain fairness and equality.
- Administers prescribed medications and treatments. Monitors vital signs.
- Demonstrates leadership ability, cooperation, and teamwork.
- Assists nursing assistants in providing direct care to residents as needed.
- Answers phone, files, and perform chart auditing.
- Assist as needed with housekeeping duties such as wiping tables, making beds, light cleaning of resident rooms and work area.
- Stocks supplies.
- May be responsible for restorative nursing care and documentation.
- Demonstrates knowledge of and interest in leadership concepts in general and specifically in nursing process, quality assurance, goal setting, and holistic approach to people. Demonstrates knowledge of government regulations for nursing homes and hospitals, and the need for implementation and continual enforcement. Demonstrates ability to direct team and non-supervisory employees in appropriate procedures. Demonstrates ability to establish and maintain effective working and public relations. Demonstrates ability to treat patient information as confidential.
MINIMUM KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
- Possess a diploma from an accredited School of Nursing with preferred one year of geriatric/rehabilitative nursing experience and an active RN license for the State of Wisconsin. Must possess the understanding and ability to comply with Federal and State rules and regulations as it applies to Nursing Home (OBRA), HSS 132, Nursing Practice Act; and maintains CPR certification.
- Yearly competencies, as well as during orientation, must be completed on Bloodborne Pathogens and Potentially Infectious Materials, Fire Safety, Customer Service, HIPAA, Geriatric Specific Care and Diversity, Hazard Communication, Hand Hygiene, Infection Control, and Caregiver Misconduct.
