Title: Community Health Worker

Job Summary: The Community Health Worker (CHW) is responsible for helping patients, their families and community members navigate and access community services, other resources, and adopt healthy behaviors. This positions primary focus is enrollment and community outreach. The CHW supports providers and Case Management staff through an integrated approach to care management and community outreach. These activities will help promote, maintain, and improve the health of patients and their family. The CHW provides social support and informal counseling, advocates for individuals and community health needs. The CHW also helps to raise awareness of affordable insurance options and provide group or one-on-one eligibility and enrollment assistance to individuals within the Center’s service area and to underinsured or uninsured patients of Center.
Essential Functions and Responsibilities:
- Participate in statewide training on navigating Michigan Medicaid and Health Exchange/Marketplace enrollment process.
- Assist patients with completing applications and registration forms.
- Conduct eligibility determination, enrollment and follow-up with uninsured patients for sliding fee discount program.
- Target outreach, education, and assistance efforts to underserved and vulnerable populations.
- Utilize Eligibility Verification Systems and various other programs to verify benefit status.
- Assist patients with updating redetermination information for continuity of benefits.
- Facilitate the selection of a Qualified Health Plan, including support for Medicaid enrollment.
- Responsible for establishing trusting relationships with patients, their families and community members while providing general support and encouragement.
- Provide support for enrollees with limited English proficiency.
- Be knowledgeable about community resources appropriate to needs of patients/families.
- Record patient care management information in the electronic medical record (training provided) and other software no later than 24 hours after patient contact.
- Participate in statewide training on navigating Michigan Medicaid and Health Exchange/Marketplace enrollment process.
- Attend community events and meetings to promote coverage options and the mission and services of Isabella Citizens for Health, Inc.
Required Education and Work Experience:
- High School Diploma or its equivalent required.
- Associates degree in health or human services is preferred.
- Experience working in a health care, insurance, community-based, or entitlement setting.
- MI Bridges, Marketplace, Advanced Directive and CHW training will be required to be completed after onboarding as well as annually.
