RN Supervisor of Service Delivery - Community and Network
Pay range: $82,899.44 - $107,371.73 annually
GENERAL STATEMENT OF DUTIES:
Under the supervision of the Chief of Health Services and Integrated Care (CHSIC), provides supervision, coaching, mentoring and first line support to nurses, medical assistants, care coordinators and other providers on health care service delivery and nursing practice within Saginaw County Community Mental Health Authority (SCCMHA). Supervises and champions our RN-led, team-based, care management philosophy of care, to provide a health care safety net for all persons served by the SCCMHA. Supervises and ensures the provision of health education, health assessments, and general whole-person health care management to persons served with the goal of improving self-management and health outcomes. Supervises care coordination processes and ensures the efficient use of clinical workflows to achieve health outcomes specific to persons served.
This position will be knowledgeable about and actively support culturally competent recovery-based practices; person centered planning as a shared decision-making process with the individual, who defines his/her life goals and is assisted in developing a unique path toward those goals; and a trauma informed culture of safety to aid persons served in the recovery process.
WORKING CONDITIONS/ENVIRONMENT:
Works in an office and community setting environment with the business pressures of dealing with time constraints and stress from long periods of computer use. Occasionally operates an automobile to attend appointments in the community and other sites, sometimes during bad weather. Close contact with the public and working with individuals with a developmental disability and/or mental illness who can be dangerous with possible injury to themselves and others.
MINIMUM REQUIREMENTS:
Education:
Associate’s degree in Nursing required. Bachelor’s degree in Nursing preferred. Advanced degree preferred
Experience:
Nursing supervisor, mentoring and/or coaching experience required. Minimum 5 years of service delivery within the Michigan community mental health system required. Medicaid system, psychiatric outpatient services, and/or specialized residential nursing services preferred. Graduate of an accredited school of nursing with at least 2-3 years of clinical experience in a medical setting preferred. Substance use disorder experience preferred.
Licenses and Certifications:
Valid Michigan Driver’s license with a good driving record. A licensed registered nurse (RN).
