- Performing comprehensive assessments, including vital sign, physical examinations, and patient history.
- Administer medications, IV fluids, and treatments as prescribed, with proper monitoring and documentation.
- Developing and implementing individualized care plans, collaborating with the interdisciplinary team to meet patient needs.
- Providing patient education and discharge planning to promote self management and adherence to treatment plans.
- Effectively communicating with physicians, other health care providers and staff and patient/families. Ensuring smooth and coordinated care.
- Maintaining accurate and comprehensive patient records, including assessments, interventions, and progress notes
