Job Title: Licensed Practical Nurse (LPN)

Reports To: Director of Nursing (DON)

Wellspring is a statewide nonprofit dedicated to serving seniors, families, and children throughout Michigan. We are currently seeking a Licensed Practical Nurse (LPN) to join our Skilled Nursing and Rehab team in Fairview. In this role, you will provide compassionate, high-quality nursing care while supporting seniors in your local community.
Responsibilities
- Provide quality nursing care that promotes resident rights, dignity, individuality, and freedom of choice.
- Conduct daily resident visits to observe, assess, and document physical and emotional status.
- Communicate effectively and respectfully with residents, families, visitors, peers, supervisors, and interdisciplinary team members.
- Keep nursing leadership informed of resident conditions, changes in status, and other relevant clinical information.
- Supervise nursing activities on the assigned unit and support staff in accordance with established policies and procedures.
- Interpret, implement, and adhere to organizational policies and procedures.
- Respond promptly to resident, family, and staff inquiries within established timeframes.
- Follow all safety and infection control protocols, including proper hand hygiene and disposal of soiled materials.
Qualifications
- Current Licensed Practical Nurse (LPN) license issued by the State of Michigan Board of Nursing (required).
- Successful completion of an accredited Licensed Practical Nursing (LPN) program.
- Minimum of two years of nursing experience in home health, medical, surgical, or critical care is preferred.
Schedule and Compensation
- Employment type: Full-time or Per Diem
- Shift: Day Shift or Night Shift, 12 hour shifts
- Weekends: Rotating weekend schedule for Full-time, required
- Pay rate: $29.50 - 32.40 per hour, based on licensure
- Night shift differential: $1.00 per hour
