RESPONSIBILITIES: This position is under the supervision of the Program Supervisor or Clinical Director and has no direct
supervisory responsibilities. This position could provide wrap around, case management and/or Supports Coordination
services to a variety of clientele including adults, adolescents, children and families requesting services. Responsibilities
include developing treatment plans using the person centered process, providing case management/supports
coordination, and making appropriate referrals. Responsibilities also include monitoring treatment plans to assess
recipient’s progress and readjusting as appropriate, carrying an active caseload, preparing case histories and needs
assessments, follow-up, and aftercare as needed.
QUALIFICATIONS: For the Outpatient Services Clinician (BSW): Bachelor’s Degree in Social Work with State of Michigan
Licensure as a Bachelors of Social Work (LBSW) or Limited Licensed Bachelors of Social Work (LLBSW). For the Supports &
Services Coordinator: Bachelor’s Degree in a Human Services Mental Health field (Such as Psychology, Sociology, Criminal
Justice, or similar nature) and one year of experience working with persons with mental illness.
