Clinical Coordinator RN
Dialysis Experience Required
Make a difference in the lives of patients with chronic kidney disease.
The Clinical Coordinator RN will work on-site at DaVita's 6-chair Skilled Nursing Facility dialysis dens. The modality eliminates the need for patient travel to an outpatient dialysis facility. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week, placing a burden on patients and adding avoidable costs to the health care system. The home site is DaVita SNF Dialysis Fenton 512 Beach St, Fenton, MI 48430.
DaVita is seeking a Clinical Coordinator RN to lead dialysis care in a skilled nursing facility. In this role, you'll oversee patient care delivery, support a clinical team, and ensure treatment meets the highest standards of safety and quality.
Key Responsibilities:
Coordinate care for ESRD patients, including labs, vitals, and outcomes
Lead and support a team of clinicians and PCTs
Deliver direct patient care in a skilled nursing facility
Monitor patient status and adjust care as needed
Ensure compliance with all clinical and regulatory standards
Support Administrative Growth
Communication with our SNF partner
Coaching teammates on the resolution of issues and escalating to Regional Operations Manager as appropriate.
Training teammates to include in-services
Managing scheduling and call-ins
Staffing the floor ~50% of the time

Qualifications:
RN license in the state of practice
Minimum of 18 months of RN experience
Minimum 6 months of dialysis experience
CPR certification
Charge RN readiness approval required
CNN or CDN certification preferred
Supervisory experience preferred
Proficiency in MS Word and Outlook
