Job Description

Contributes to the health assessment of each patient.
Provides direct patient care under supervision of the Provider.
Administers injections and vaccines as requested.
Follows protocols for various patient appointment types including following standing orders.
Communicates effectively with patients, family and the entire health care team.
Promptly responds to voice messages and portal messages throughout the day.
Schedules and/or tasks patient referrals and testing as appropriate.
Retrieves patient test results in a timely manner for labs, radiology, etc. and communicates results with the Provider.
Maintain tracking records/reports of referrals and testing to ensure orders are reconciled.
Recognizes and reacts appropriately to an urgent or emergent situation.
Maintains accuracy and documentation of patient records and information in the EHR system.
Appropriately documents all interventions in patient care.
Completes competency assessment as required.
Maintains professional legal/ethical behavior.
Maintains a safe and therapeutic environment for the patient.
Follows all health center policies including privacy policies and Infection Control policies.
Participates in community outreach events and community health focused clinics as needed.
May assist providers with Telehealth as needed.
Keeps workstation organized and strives to keep all patient protected health information confidential in work area.
Schedules are subject to change at any time based on the provider’s schedule adjustments.
Performs other duties as assigned by Provider and direct supervisor within scope of education and training.
