General Purpose of Job:
Provides direct physical therapy services under the direction and supervision of a licensed Physical Therapist according to the physician approved written plan of care.
Essential Duties:
This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
- Provides direct physical therapy treatment to patients according to instruction of Physical Therapist, and in accordance with the written plan of care
- Observes patient and reports to Physical Therapist and department personnel the patient’s reaction to treatment and any changes in patient’s condition
- Instructs patients, family and/or caregiver in proper techniques to assist patient in achieving maximum level of independence
- Maintains necessary records, progress notes, and conference notes, which are incorporated into the patient’s clinical record within established time frame
- Maintains professional standards, compliance with State, Federal (CMS conditions of participation) and Joint Commission regulations, and infection control protocol by participating in in-services, staff meetings, process improvement teams, continuing education seminars when available, and adhering to the SHC expectations – standards of care guidelines

• PTA license to practice in State of Michigan • Valid driver's license and automobile insurance • Active American Heart Association BLS Certification upon hire |
| Work Experience |
• Two years acute care or home care experience |
| Education |
• Graduate of a Physical Therapy Assistant program |
| Specialized Knowledge and Skills |
Required: • Demonstrated ability to work with patient, caregivers, and others in a variety of home care situation • Demonstrated interpersonal skills to effectively and appropriately communicate with patients, families, physicians, and interdisciplinary team • Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job • Demonstrated ability to assemble and document data in proper format • Ability to enter/exit patient homes that may/may not be handicap accessible Preferred: • Home Care experience |
