This remote role requires an active RN license in Michigan and a Compact Nursing License that includes Florida RN privileges. Job Description: * Conduct medication reconciliation and follow-up care for high-risk patients post-discharge. * Perform structured telephonic clinical assessments using motivational interviewing techniques. * Document patient encounters in real time using Care Convene while managing calls via a third-party phone system. * Communicate professionally with patients experiencing emotional or psychological barriers to care. * Maintain accurate and timely clinical documentation in electronic health records. * Escalate complex care management cases based on clinical criteria and protocols. * Maintain patient privacy with a quiet, secure home workspace and reliable internet connection. * Use Slack, Flex phone, and required tech tools; training and support provided during onboarding. * Adhere to HIPAA compliance, participate in ongoing quality evaluations, and work weekend shifts as needed.
