Requirements
- Must be licensed to practice as a Nurse in the State of Michigan and 1- 3 years clinical experience working with skin assessment and treatment is preferred
- Must be currently certified in wound care or become certified within 6 months of hire.

Summary
- Total Body Assessments are completed weekly and PRN by unit nurses on every resident in LTC. The Treatment Nurse is notified of abnormal findings of pressure ulcers and is responsible for follow-up assessment/treatment and for pressure ulcers initial documentation. A weekly documentation will be completed by Treatment Nurse for any resident with a pressure ulcer to include progress, treatment, interventions and/or recommendations from the Skin Committee.
- Weekly pressure ulcer reports will be completed on all residents with staged pressure ulcers (includes Stage 1 thru Stage IV) and non-pressure ulcers skin conditions and will be distributed to the Neighborhood Manager for review. A copy of this report will also be sent to the dietician.
- Completes weekly and PRN assessment on all residents with pressure ulcers. Documentation to be completed on pressure ulcer flow sheet. All other skin conditions will be evaluated by the Treatment Nurse upon referral and follow-up care will be provided as appropriate.
- The Treatment Nurse will document any specific skin problems on the care plan, notify the dietician and adjust the resident care cards to affect the Certified Nursing Assistant (CNA) assignments.
- Appropriate interventions will be initiated on any staged skin breakdown according to the medical staff protocol.
- The Neighborhood Manager and Physician will be notified of these interventions or of any resident concerns within 24 hours.
- Weekly dressing changes and measurements will be completed by the Treatment Nurse on all Stage 2, 3 and 4 pressure ulcers/wounds. Daily dressing changes will be completed by the unit staff nurse. Stage 1 pressure ulcers treatment will be completed by the Licensed Nurse on the unit.
- Ensure all documentation is complete according to policy.
- Works with Restorative Nursing and floor staff to educate on skin maintenance.
- Provides reports as requested to the Neighborhood Manager.
- Responsible for a complete and thorough knowledge of OBRA guidelines related to skin prevention.
- Perform any other duties as assigned by the Director of Nursing.
- Required to participate in Performance Improvement program as needed.
- Practices standard precautions, utilizing hand washing and barrier use when there is a risk of contact with blood, body fluid, mucous membrane or non-intact skin. Provide barrier for self when there is evidence of non-intact skin on hands.
- Regular and punctual attendance is required.
- Compliance with mandatory overtime required when necessary.
- Other duties as assigned.
