Position Overview: We are seeking a skilled and empathetic Medicare Admission/Visit Nurse to join our dynamic team. The ideal candidate will play a pivotal role in ensuring that our clients receive the best possible care from the moment they enter our program.

Key Responsibilities:

• Conduct initial assessments and intake processes for new Medicare clients.

• Develop and implement individualized care plans based on client needs.

• Serve as a liaison between clients, caregivers, physicians, and other healthcare professionals.

• Ensure compliance with Medicare guidelines and regulations.

• Provide ongoing support and follow-up with clients and their families.

• Maintain accurate and detailed documentation of all client interactions and care plans.

Qualifications:

• Current and valid RN license in the state of Michigan.

• Prior experience in home health care or a related field preferred.

• Strong understanding of Medicare regulations and guidelines.

• Excellent communication and interpersonal skills.

• Compassionate, patient, and dedicated to providing exceptional care.

Pay: $36.00 - $38.00 per hour
