Opportunity
Our company is currently seeking a dedicated Registered Nurse Case Manager to join our team in providing exceptional patient care. There is both full and part-time positions available located in the Metro Detroit area. Must have Monday- Friday daytime availability to assist patients. As a Registered Nurse Case Manager, you will play a crucial role in coordinating patient care and ensuring optimal outcomes. Must be local to the Metro Detroit area and willing to travel locally.

Responsibilities

• Strong utilization management skills to assess patient needs and develop individualized care plans
• Coordinate with interdisciplinary teams to ensure effective inpatient care transitions
• Apply medical terminology and physiology knowledge to enhance patient understanding of treatment plans
• Collaborate with healthcare providers in various specialties including OR, hospital medicine, emergency medicine, primary care, and critical care
• Advocate for patients' needs and facilitate access to necessary resources for comprehensive care

Qualifications

• BSN or Associates and 5+ years of experience
• Certified case manager a plus
• Active Registered Nurse license in Michigan
• Proficiency in utilization management practices and patient care coordination
• Strong understanding of medical terminology and physiology
• Excellent communication skills and ability to work effectively within a multidisciplinary team
• Experience in hospital settings preferred
