Job Description:
We are seeking a compassionate and dedicated individual to join our team as a Community Mental Health Home Health Aide. In this role, you will provide essential support and assistance to individuals with mental health needs within their homes and community settings. Responsibilities:
- Assist clients with activities of daily living, including personal hygiene, grooming, dressing, and meal preparation.
- Provide emotional support and companionship to clients, fostering a supportive and nurturing environment.
- Monitor clients' mental and physical well-being, reporting any changes or concerns to the appropriate healthcare professionals.
- Implement and follow individualized care plans developed by mental health professionals, ensuring that clients receive comprehensive and personalized care.
- Accompany clients to appointments, outings, and recreational activities, promoting social engagement and community integration.
- Document observations, interventions, and progress notes accurately and timely in accordance with agency policies and procedures.
- Collaborate with interdisciplinary team members, including nurses, therapists, case managers, and family members, to coordinate care and support client goals.
Requirements:
- High school diploma or equivalent.
- Previous experience working in a mental health setting or with individuals with mental health challenges is preferred.
- Certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) is desirable.
- Compassionate and empathetic attitude with a genuine desire to help others.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and respect the privacy of clients.
- Reliable transportation and valid driver's license.
- Ability to pass background checks and drug screening.
- Willingness to undergo additional training and professional development as required.
