Join our team as an Admissions Coordinator Nurse, where you’ll play a vital role in guiding referrals through the full admission cycle and creating a smooth, welcoming transition for new residents and their families.
In this collaborative and impactful role, you will:
- Manage and coordinate referrals from initial clinical review through admission, ensuring timely and clinically appropriate placement
- Maintain an accurate waiting list and referral status tracking
- Conduct clinical evaluation of referrals, including:

- Diagnosis review for skilled and Medicaid eligibility criteria
- Medication analysis for availability and nursing clinical capabilities
- Provider order transcription and EHR data entry
- Serve as a liaison between hospitals, families, providers, referral partners, and internal interdisciplinary teams
- Provide facility tours and education to prospective residents and their representatives
- Coordinate admission day processes across Social Services, Business Office, and Nursing for a seamless and person-centered experience
- Process hospital bed holds and re-admissions
- Partner with the Business Office to support occupancy and facility goals
- Identify barriers to admission and recommend solutions to nursing and social service leadership
- Support the development and improvement of admissions procedures, reports, and policies
- Maintain regular, on-site, punctual attendance
You’re a great fit if you bring:
- An active, unencumbered nursing license in Michigan (RN or LPN)
- Experience in long-term care, skilled nursing, or care transitions (preferred)
- Strong communication, organization, and critical thinking skills
- Understanding of insurance, billing, and coverage types related to admissions
- Ability to navigate family and system dynamics with empathy and professionalism
- A passion for building relationships and supporting residents and families during transitions of care
