Title: Outpatient Therapist
Supervisor: Director of Behavioral Health
FLSA Status: Salary - Exempt
Job Summary: The Outpatient Therapist is responsible for providing a wide range of mental health services to facilitate individual psychosocial adjustment for people with mental illness and developmental disabilities to ensure meaningful participation in community life. This includes individual therapy with children, teens, adults, families and maternal infant therapy. The Outpatient Therapist is a member of the Center’s integrated care team.
Essential Functions and Responsibilities:
· Evaluation and diagnoses of consumers (clients) from all Center locations.
· Provides individual, family & group therapy based on the person-centered plan and best practice standards.
· Provides assessments, consultation, referral and treatment coordination, as appropriate.
· Provides care management service in collaboration with the physician for assigned patients with behavioral health diagnosis in need of additional consultation and monitoring.
· Provides coordination of services with other Center providers and partnering community agencies.
· Maintains patient/consumer care hours consistent with agreed upon schedule with the CEO.
· Participates in Center and community activities related to prevention, awareness and treatment of mental health and substance use disorders.
· Participates in after-hours call for behavioral health consumers and provides crisis intervention when appropriate.
· Closing Charts - completes all necessary documentation within 72 hours of consumer’s scheduled and kept appointment in order to bill for services in a timely manner.
· Remains current with professional credentialing and knowledge base with the scope of practice.
· Participates in committees and professional development activities.
· Maintain knowledge about and actively support: 1) person-centered planning, 2) culturally competent practices; and 3) compliance and process improvement.
· Schedule patient appointments for outpatient therapy.
*Participates in community outreach events as needed.
*May provide services at various health center sites.
· Provides Telehealth appointments in addition to face-to-face, from an approved location.
· Responds appropriately to emergent situations.
Qualifications:
Required
- Master’s degree in social work or counseling or clinical (LMSW or LPC).
- Current Michigan Licensure for such credentials.
- Experience working with children and families as a therapist.
Preferred
- Experience working with low-income individuals, homeless and/or at-risk populations;
- Experience working in a mental health or medical care setting.

- Ability to work efficiently in the Center’s EHR to facilitate proper documentation and billing;
- Ability to write routine reports and correspondence;
- Ability to establish rapport with clients, colleagues and community agency representatives;

- Able to follow oral and written instructions;
- Possesses flexibility, personal integrity and the ability to work effectively with clients, their family members, staff, and community agencies;
- Working knowledge of Microsoft Word, Excel and E-mail.
