Community Health Worker

• $20.06/hr
• This is a temporary, part-time position working a maximum of 1,000 hours per calendar year. There are no benefits.

Duties:

• Supports improved access to care and better health outcomes by providing outreach, education, and advocacy across Berrien County Health Department programs.
• Provides health promotion and prevention education, delivering culturally responsive services, and reducing barriers to health and social supports.
• Plans and delivers individual and group education through workshops, home visits, phone calls, and community-based settings.
• Collaborates with staff and community partners to provide health promotion and prevention education on topics such as chronic disease, behavioral and substance use health, parenting, sexual health, nutrition, and social-emotional well-being.
• Conducts client interviews using basic motivational interviewing and goal-setting techniques, offers general support and encouragement, and serves as an advocate by connecting individuals and families to appropriate medical and social services.
• Participates in outreach activities, community assessments, health screenings, and presentations at local agencies and events. Responsible for maintaining accurate documentation, data entry, and records required for medical files and grant reporting.

Minimum Requirements:

• High school diploma with college experience preferred.

Desired:

• Certification as a Community Health Worker.
• Experience interacting with high risk individuals in a public health-related field.

Special Requirements:

• Must possess and maintain a valid driver’s license throughout the course of employment.
• Non-traditional work hours may be required on occasion.
• In times of a public health emergency, may be required to report for specialized assigned duties inside or outside of Berrien County.

Required Knowledge and Skills:

• Strong verbal, written, presentation, and interpersonal communication skills to effectively share information and build trust with clients and community members.
• Ability to manage time efficiently, prioritize multiple responsibilities, and document services in accordance with best practices.
• Proficiency in Windows-based systems and Microsoft Office applications.
• Knowledge of client confidentiality and client rights in compliance with the Health Insurance Portability and Accountability Act (HIPAA).
• Demonstrated sensitivity to diverse socioeconomic, cultural, ethnic, and health-related needs.
