Grandvue Medical Care Facility—Charlevoix County’s premier long-term care provider—is seeking a dynamic, resident-centered, and quality-driven Registered Nurse to lead our Restorative Nursing Team. This role is built on current standards of care and evidence-based practice, offering an exciting opportunity for an outgoing and autonomous RN passionate about helping Residents achieve or maintain their highest practicable level of function.
Position Summary:
The Restorative Nursing Coordinator leads restorative programming and the Restorative CNA team. They work closely with contracted therapy providers and interdisciplinary team members to promote Resident independence and optimal functional outcomes. This role is ideal for a motivated RN who enjoys taking initiative, working independently, and fostering a culture of person-centered care.
The successful candidate will:
- Provide autonomous leadership of the Restorative Nursing program using evidence-based, individualized care practices.
- Supervise, mentor, and support Restorative CNAs to ensure consistent, high-quality resident care.
- Utilize strong communication, critical thinking, and root cause analysis to assess needs and improve processes.
- Work collaboratively with skilled therapy partners to maximize Resident outcomes and support continuity of care.
- Actively participate in interdisciplinary team meetings and contribute to comprehensive care planning.
- Develop, implement, and evaluate restorative programs aligned with regulatory expectations and current standards.
- Build and maintain positive, professional relationships with Residents, families, and staff.
- Promote a culture of functional independence, active living, and person-centered care throughout the facility.
Qualifications:
- Current Registered Nurse (RN) license in the State of Michigan.
- Experience in restorative nursing or long-term care preferred.
- Strong leadership, supervisory, and communication skills.
- Emotional maturity and professionalism in all interactions.
- Demonstrated critical thinking, assessment, and problem-solving abilities.
- Familiarity with Minimum Data Set (MDS) requirements.
- RAC-CT certification or willingness to obtain.
- Commitment to continuing education, evidence-based practice, and professional growth.
