Job Summary:
The Life Services Life Coach II provides one-on-one coaching and development for participants in the Life Services program with a focus on complex populations as defined by member needs both urgent, immediate, and long term to prepare members for employment or education programs and assist them with social determinants of health (SDOH) needs.
Essential Functions:
Complete required assessments by gathering information from the member
Support a positive environment for the member by motivating, recognizing and engaging participants through proper communication and individualized practices
Maintain required documentation in case management system and follow all e-manual instructions
Research developments in the fields of workforce and career development for those in and working to get out of poverty
Act as a liaison to social service agencies (SDOH), government agencies, employers, employment and education programs
Advocate for Life Services participants to access resources
Coordinate logistics for appointments as necessary
Support development of Care Plan and monitoring and reporting on participant status
Arrange and coordinate services with community leaders; partners to maximize utilization of available resources and support groups
Work with CM to endure Care Plan is carried out appropriately
Regular travel to conduct member, provider and community-based visits as needed to ensure effective administration of the program
Maintain a comprehensive understanding of Life Services best practices, as demonstrated via supervisor audits, successful goal setting and achievement, and skills assessments
Required to carry the minimum caseload set by supervisor
Apply coaching skills to complex populations (e.g., reentering citizens, SUD, SBH) through ongoing learning (e.g., attending multiple training courses) and sustained focus on a particular population
Required to perform additional tasks and projects, such as support new hire onboarding, training, and mentoring when applicable
Perform any other job duties as requested
Education and Experience:
Associate's degree in Social Work, Human Services, or Nonprofit Management or equivalent years of relevant work experience is required
Bachelor's Degree in Social Work, Human Services Administration, or non-profit Management is preferred
A minimum of three (3) years of social service delivery experience is required
A minimum of two (2) years of Life Services tenure or relatable experience is required
Experience working with populations in poverty is required
Experience with Motivational Interviewing is required

Intermediate proficiency in Microsoft Office Suite to include Word, Excel and PowerPoint

Licensure and Certification:
Coaching or other certification or licensure related to position is preferred. Examples of licensure or certification can include but not limited to social work, chemical dependency, workforce development, re-entry, IDD, mental/behavioral health.
Training/Working with population in poverty is preferred
Must have valid driver's license, vehicle and verifiable insurance. Employment in this position is conditional pending successful clearance of a driver's license record check and verified insurance. If the driver's license record results are unacceptable, the offer will be withdrawn or, if employee has started employment in position, employment in the position will be terminated.
