Under the supervision of the Children’s Special Health Care Services/Child Lead, Nurse Family Partnership, or Maternal Infant Health Program Supervisor, the Public Health Nurse (PHN) addresses community needs by defining, planning, implementing, and evaluating the public health nursing services to be rendered primarily to families beneath the income threshold for WIC. The PHN provides comprehensive and professional public health nursing assessments and services to children, pregnant persons, families, and/or the community under one of the following programs: Children’s Special Health Care Services, Child Lead, Maternal Infant Health Program, or Nurse Family Partnership. The PHN may also perform functions in aid to specific programs such as the Fetal Infant Mortality Review (FIMR) and / or the Child Death Review (CDR).

Essential Functions

1. Visits individuals and families in their homes, schools, offices, or other community sites to assess physical, emotional, social, and environmental needs, in relation to healthy development and life course development.
2. Establishes nurse-client rapport, while also promoting client autonomy, self-esteem, and problem-solving skills.
3. Promotes health literacy in the community through the development of evidence-based and current health education materials and group teaching events on topics such as decreasing lead exposure, child birthing, and car seat safety.
4. Engages in interprofessional collaboration with members involved in client circles of care.
5. Assists with identifying and establishing specific, measurable, achievable, relevant, and timely goals that promote health and wellness.
6. Implements a tailored plan-of-care to achieve targeted goals, in applicable areas such as decreasing lead exposure, parenting, post-partum care, nutrition, child health care, family planning, life course development and special health care.
7. Monitors progress towards targeted goals, and reviews plan-of-care on an on-going basis, making modifications as needed.
8. Facilitates needs-based referrals to community agencies and/or resources.
9. Serves diverse clientele, including families that primarily speak languages other than English.
10. Participates in Continuous Quality Improvement projects as required by specific programs.
11. Engages in provider education and outreach.
12. Uses data to drive appropriate interventions in order to achieve the best client health outcomes.
13. Interfaces with State of Michigan staff in order to collaborate to perform all necessary services and also to ensure all required State of Michigan program requirements are met.
14. Abides by nursing documentation best practice guidelines and records all nursing activities in a timely manner.
15. PHN will engage in opportunities that promote professional development and continuing education.
16. Performs case extraction services for FIMR and/or CDR, which may involve contacting providers to review medical records, interviewing community services that families engage with, or performing interviews of families who have experienced infant or child loss.
17. Presents case extraction information to FIMR and/or CDR committee for review to establish future recommendations for infant and child death prevention.
18. Participates in FIMR and/or CDR committees to assist in establishing death prevention recommendations.
19. Performs public policy education and advocacy under program guidance.
20. Participates in meetings and collaborates professionally with various community organization groups such as Local Leadership Group or Great Start Coalition.
21. Sits on various committees within their specified program or within Ingham County to influence positive growth and change for the organization(s).
22. Educates nursing students on aspects of public health nursing. This may include having a student shadow a home visit, sit in on group education event led by the PHN, or other opportunities as deemed appropriate by the PHN.
23. Ensures that client records are kept secure and confidential and maintained consistently with Health Department policies and procedures and HIPAA standards.
24. During a public health emergency, the employee may be required to perform duties similar to but not limited to those in his/her job description.

Employment Qualifications

Education: Possession of a Bachelor’s Degree in Nursing.

Licensure: Possession of a current, unrestricted license to practice as a Registered Nurse in the State of Michigan.

Experience: At least 2 year of nursing experience.

Desired Qualifications:

• Possession of a vehicle and current auto insurance or other dependable travel throughout Ingham County.

• Possession of valid driver’s license in the State of Michigan, or equivalent.

• Possession of a valid BLS certificate.

Requirements and Working Conditions

Physical/Technological Requirements:

• Ability to access office files.

• Ability to enter and retrieve information from computer.

• Ability to access charts, other records, and documents of the department.

• Ability to operate copy machines and other office equipment.

• Stooping, kneeling, and crouching in order to perform assessments and/or testing.

• Requires the ability to travel throughout the county to various locations and complete home visits.

• May require the ability to lift and carry equipment weighing up to 50 lbs.

Working Conditions:

• Works in office and approved alternative off-site locations in the county.

• Risk of exposure to communicable diseases, blood, and other bodily fluids.

• Risk of exposure to biological hazards i.e. insect infestations. 

• This position works in both an indoor and outdoor environment.  

• This position is provided with personal protection equipment, which should be utilized based on nursing clinical judgement to minimize risks.
