Opportunity
Currently seeking a dedicated Registered Nurse Case Manager to join a seasoned team in the Marquette, MI area providing exceptional patient care. This part-time position will start with approximately 10 hours per week. Must have Monday- Friday daytime availability to assist patients and accompany to appointments and home visits if needed. As a Registered Nurse Case Manager, you will play a crucial role in coordinating patient care and ensuring optimal outcomes. Must be local to the Orlando area and willing to travel locally.

Responsibilities
- Strong utilization management skills to assess patient needs and develop individualized care plans
- Arranging and attending physician appointments
- Assistance with locating and navigating the transition to alternate living environments (such as ALFs)
- Assisting with insurance related issues including, understanding the client's medical coverage and aiding in resolving billing issues.
- Coordinate with interdisciplinary teams to ensure effective inpatient care transitions
- Medication set up and management
- Collaborate with healthcare providers in various specialties including OR, hospital medicine, emergency medicine, primary care, and critical care
- Advocate for patients' needs and facilitate access to necessary resources for comprehensive care
Qualifications
- BSN and certified case manager required
- Active Registered Nurse license in Florida
- Minimum of 4 years of experience in case management or related field
- Proficiency in utilization management practices and patient care coordination
- Strong understanding of medical terminology and physiology
- Excellent communication skills and ability to work effectively within a multidisciplinary team
- Experience in hospital settings preferred
