Key Responsibilities
- Conduct behavioral and developmental health screenings and assessments.
- Support patients and families in managing diagnoses and accessing resources.
- Provide brief interventions to assist with behavioral and lifestyle changes.
- Collaborate with healthcare providers to develop patient-centered care plans.
- Maintain accurate and timely documentation for all behavioral health services.
- Advocate for patients and connect them with relevant community services.
What We're Looking For
- Education & Licensure:
- Master's degree in social work (MSW)
- Active LMSW license (Michigan)
- Experience:
- 3 + years in social work (preferably in primary care, outpatient, or hospital settings)
- Certifications:
- CPR Certification (American Red Cross or AHA)
- Perinatal Mental Heatlh Certification (or willingness to obtain within 6 months of hire)
- Skills:
- Strong patient advocacy, case management, and collaboration abilities
