Requirements
- Graduation from an accredited school of nursing and holding a current license to practice nursing in the State of Michigan as a registered nurse.
- Three to five years clinical experience at HSS or experience in a clinical setting related to specific areas of this position. Has knowledge of Federal, State and other regulatory agencies pertinent to clinical area.
- Must possess interpersonal skills to interact positively and effectively with a variety of residents/families/visitors, physicians, other disciplines and be able to maintain a positive milieu and employee morale within the assigned Neighborhood(s).
- Must possess nursing knowledge necessary for effective formulation of patient care plans for residents in unstable condition, implementation of care and appropriate documentation of pertinent information regarding resident’s condition and resident outcome.
- Work requires the analytical ability to identify problems, plan for their resolution, monitor and evaluate outcome.
- Demonstrates commitment to continuing professional development through continuing education, committee work, etc.
- Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the residents served on his/her assigned Neighborhood(s). Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the resident’s status and interpret the appropriate information needed to identify each resident’s requirements relative to his/her age-specific needs, and to provide the care needed as described in the Neighborhood’s/department’s policies and procedures.
- Knowledge of RAI (Resident Assessment Instrument), Federal and State (OBRA) Regulations.
- Basic computer knowledge.
Summary
- Responsible and available for twenty-four hour care, safety and management for all residents on assigned Long Term Care Neighborhood(s) through coordinating the development, implementation and evaluation of the nursing regimen, promotion of safety practices and employee direction and supervision.
- Confers with physicians, other professionals, and other departments for provisions of total care.
- Responsible for meeting requirements of MDCH and other accrediting/regulatory agencies for resident care and results-oriented documentation.
- Responsible for an ongoing Performance Improvement program for the Neighborhood(s) (i.e. developing, monitoring, evaluating care and developing action plans for identified problems).
- Initiates and enforces the development, maintenance and interpretation of standards of nursing practice to ensure safe, therapeutically effective nursing care.
- Reviews and analyzes trends pertinent to delivery of nursing care services within assigned nursing Neighborhood(s) and initiates appropriate innovations/changes in consultation with the Director of Nursing.
- Assumes responsibility for facility supervision as necessary.
- Promulgates the established philosophy and objectives of HSS and the nursing department.
- Assists in developing and implementing goals and objectives for assigned Neighborhood(s).
- Interprets legal nursing responsibilities as indicated by facility and Nursing Policies, the Michigan Public Health Code and the Practice of Nursing as indicated in the Code.
- Provides leadership in clinical area and in developing staff to meet competency/certification/advanced education in specialty of the Neighborhood(s) .
- Complies with the code of ethics for nurses.
- Responsible for nursing employees on own Neighborhood or other Neighborhoods through:
- Interviews job applicants in coordination with the Director of Nursing.
- Provides orientation and evaluation of new employees.
- Assesses subordinate’s performance and competencies, initiates counseling, provides written performance evaluations and initiates disciplinary action per policy and procedure.
- Evaluates and makes provision for staff education, as indicated.
- Maintains good communication with staff (individually and in groups).
- Makes daily rounds to evaluate effectiveness of care delivery and performance of subordinates to assure compliance with Performance Improvement standards.
- Performs other duties as assigned by the Director of Nursing.
- Practices standard precautions, utilizing hand washing and barrier use when there is a risk of contact with blood, body fluid, mucous membrane or of non-intact skin. Provides barrier for self when there is evidence of non-intact skin on hand.
- Facilitates and assures that all resident’s care conferences are in compliance with Federal/State regulations and meet HSS policies and procedures.
- Establishes and follows up on routine reviews of current goals/approaches for both effectiveness and success, collecting information for evaluation, and acknowledging achievements with resident, significant others, and care planning team.
- Consistent review of schedule of MDS, CAAs CAA Summaries, care plans, consents, discharge planning, resident/significant other education to assure accuracy and completion.
- Attends nursing meetings, in-services and seminars as directed.
- Keeps current on all RAI, care planning updates.
