Ensure the highest degree of quality care is delivered at all times in all areas of nursing including staff development and infection control, MDS, wound and skin care and restorative.

Work with the administrator and director of nursing, professional nursing staff and therapists in directing the overall operation of the nursing and therapy services in accordance with current federal, state, and local standards, guidelines, and regulations that govern the facility.

Ensure an environment which encourages each resident's ability to maintain or attain the highest level of independence and physical, mental, and psychosocial well-being

Maintain the comfort, privacy, and personal dignity of each resident in the delivery of services to them Interact with residents in a manner that displays warmth

Supports the DON and Wellspring management in clinical and operational needs

Tracks community acquired infections; map and track infections geographically and related to contacts

Collaborates with DCS and Medical Director to problem-solve and prevent transmission of infections of residents and employees. High level focus on VRE, MRSA & C-difficile

Provides education as needed to ensure control of transmission and appropriate treatment of infections

Oversees TB exposure program for residents, staff and volunteers and "FIT" process
-
Coordinates the IV Team to ensure continuity of resident centered care

Leads orientation process for new employees
-
Assists with development of skills fairs and other approaches to staff development and education

Ensures that yearly education is completed as required by regulations for all employees

Assists DON and other nursing department management staff with researching best practice and developing educational presentations on specific approaches needed to respond to resident care requirements

Formulates effective working relationships and communication with staff, patients, families and other customers

Participates in the Quality Assurance and Performance Improvement committee, and assist in developing and implementing appropriate plans of action to improve performance

Reviews EHR periodically to assure proper and timely documentation being done

Assures documentation reflects the resident's status, indicating progress, maintenance, or decline; must ensure that a resident's abilities in activities of daily living do not diminish unless circumstances of the resident's clinical condition demonstrate that decline was unavoidable

Documents and reviews on 24 hr. report daily. Identify any pertinent issues and/or changes; assure that each shift follows through with needed documentation. Ensure physician and family are notified as per policy

Acts as a resource for staff and provide on-going staff education

Maintains high regard for confidentiality

Attends/participates in departmental meetings and care conferences

Participates in 24-hour on-call rotation

Designs QA processes and Improvement programs to ensure compliance based on MDS findings

Collects an analyses data for diverse sources in order to problem solve for appropriate decision making

Maintains professional standards by actively participating in continuing education activities, in-service education

Participates in orientation, supervision, training and evaluation of students and other staff

Prepares and is ready for regulatory reviews: State; Fire Marshal, etc.

Observes, monitors and evaluates outcomes, customer satisfaction and resident rights integration. Suggests interventions to enhance resident rights.

Oversees RAI-MDS, CAAS, Care Plan responsibilities

Assesses complex patient wounds when they develop or when residents are admitted with an existing wound

Assesses and evaluate wound associated pain prior to treatment
-
Performs wound treatment utilizing infection control techniques

Collaborates plan of care with nursing staff, dietician, and physician

Stages and measures wounds and document per protocol. (not only pressure but documentation on skin conditions other than wounds) Assess all new/readmissions for potential skin/wound concerns, within 24 hours in conjunction with admitting nurse

Provides individualized attention, which encourages each resident's ability to maintain or attain the highest level of independence and physical, mental, and psychosocial well-being
