Job Title: RN – Case Manager
Location: Remote, MI
Duration: 6-12 months with possible extension
Shift: 8am-5pm EST (Mon-Fri)
Summary:
The Case Manager RN leads a multidisciplinary team to provide personalized care management to a diverse population with various health and social needs. Serving as the primary contact for members, caregivers, and providers, they utilize phone, email, text, and online messaging platforms. The Care Manager RN oversees the development, implementation, and monitoring of care plans to optimize health outcomes, in partnership with providers and community resources.
Qualifications:
- Required: Nursing Diploma or Associate’s degree; active Michigan RN license.
- Preferred: 3+ years clinical experience; 1+ year case management in a managed care setting.
Skills & Abilities:
- Strong critical thinking, problem-solving, and decision-making skills.
- Empathetic, supportive, and skilled in motivational interviewing.
- Proficiency in time management and multi-tasking with various systems/tools.
- Intermediate computer and Microsoft Office proficiency (Excel, Word, Outlook, etc.).
- Excellent communication and interpersonal skills, both written and verbal.
